The NKF Integrated Care Team brings together a diverse group of healthcare professionals to provide care tailored to each patient’s multifaceted needs.
Kidney failure is a life-altering condition that affects not just physical health of a patient but also aspects such as financial stability, psychological well-being and social relationships. Recognising that the needs of patients are multifaceted and require more than just medical treatment, The National Kidney Foundation (NKF) does not subscribe to a one-size-fits-all approach. The Foundation has developed a comprehensive approach to community dialysis care that extends beyond traditional methods.
Supporting Every Aspect of a Patient’s Journey
NKF’s Integrated Care Team (ICT) brings together a diverse group of healthcare professionals, including renal doctors, nurses, social workers, clinical psychologists, dietitians, therapists, and exercise specialists. This multidisciplinary team works collaboratively to provide person-centred care that addresses the unique needs of each patient.
Tim Oei, NKF Chief Executive Officer, emphasises the importance of this approach. “To many kidney failure patients, dialysis can feel like the end of the world. We want them to know that this is not the case. They can live with hope, fulfilment and dignity, especially with support from those who journey alongside them. Helping them to find and achieve meaning in their lives—this is the type of social impact NKF is committed to creating.”
The impact of this integrated approach is clear. A case in point is a 65-year-old female patient who was struggling with various health issues. Through the combined efforts of NKF’s social worker, dietitian, nurse, and occupational therapist, they were able to address her malnourishment, mobility challenges, and ensure smooth dialysis treatments. In less than two years, her health showed significant improvement, demonstrating the effectiveness of NKF’s holistic care.
NKF’s community dialysis care model aims to go beyond treating kidney disease—it empowers patients to thrive in their communities. The dedicated professionals work tirelessly to inject hope, dignity, and a sense of purpose into patients’ lives, showing them that dialysis is not the end. Instead, it is an opportunity for renewed hope and meaning.
Where Every Role Matters
At NKF, patients are at the heart of everything. Every role, despite its diversity, is dedicated to the core values of Nurturing, Integrity, Compassion, Excellence and Respect in the pursuit of quality service and remarkable healthcare standards.
For instance, nurses at NKF serve as the patients’ primary point of contact, providing day-to-day health management and monitoring progress. They are attuned to patients’ overall needs and can alert other team members when issues arise.
Therapists play a crucial role in understanding how various medical conditions affect a patient’s daily activities. Physiotherapists focus on improving mobility, balance, and strength, while occupational therapists help patients regain the ability to perform daily activities.
Social workers offer holistic support by addressing social, emotional, and financial challenges. They conduct psychosocial assessments, counselling, connect patients with community resources, and coordinate care plans.
Dietitians conduct comprehensive renal dietary assessments for patients, providing personalised advice tailored to individual nutritional markers and lifestyle considerations. They also prescribe nutritional supplements when necessary to ensure optimal health.
This integrated team effort, along with close collaboration with hospitals and community partners, ensures a seamless, effective, and efficient continuum of care to meet the whole spectrum of a patient’s health needs. With a collective goal to deliver compassionate, effective, and holistic dialysis care, NKF’s community dialysis care model aims to help patients achieve better well-being and quality of life, empowering them to remain and thrive in their communities.
Investing in the Future
NKF is committed to nurturing the next generation of healthcare professionals through sponsorships and study awards. For instance, their Enhanced Sponsorship for nursing students covers two years of tuition fees, provides a one-time allowance, and offers a monthly stipend.
Su Yin, an NKF dietitian, received the first NKF sponsorship for a Bachelor of Science (Honours) in Dietetics and Nutrition from the Singapore Institute of Technology. Likewise, Qi Huang, an NKF nurse, benefited from the Enhanced Sponsorship for his Bachelor of Science (Honours) in Nursing from the National University of Singapore, which fully covered his tuition fees and provided monthly allowances.
Sharon’s journey also reflects NKF’s commitment to fostering growth and development within its ranks. Her experience as a caregiver for several years inspired her to shift from a career in accounting at a multinational company to becoming a social worker at NKF. Five years later, Sharon now serves as a Zone Lead, overseeing 14 dialysis centres. Her story exemplifies how NKF empowers its employees to find their true calling in service of others.
Fauzan, an experienced occupational therapist within both acute and community settings, embodies the pivotal role of occupational therapy in the lives of patients with kidney failure and kidney disease. He is passionate about exploring innovative possibilities within the field to further enhance patient care.
NKF is committed to delivering social impact through compassionate, effective, and holistic dialysis care.
